Québec residents

To submit a request, follow these steps:

  1. Complete and sign the Access to Information on Vaccines Request form.
  2. Send the form, along with a photocopy of a valid I.D. document (health insurance card or birth certificate) to your regional officer in charge of requests, by one of these ways:
    • Post mail
    • Fax
    To find contact information of your regional officer, consult the list of officers in charge of requests for access to information on vaccines.

You may contact your integrated health and social services centre (CISSS) or your integrated university health and social services centre (CIUSSS) to receive a printed version of the form.

Access to information on vaccines requests, and responses to them, will not be dealt with by telephone.

If you discover that the information is incomplete, you may request access to information from each establishment that administered vaccines.
